SIGN UP IS EASY!
A COUPLE OF IMPORTANT NOTES...
You need to sign up by OCTOBER 31, 2020, in order to have CenturyLink install fiber optic cable to your premise as part of Boulder Junction's High Speed Broadband expansion projects for Phase 1 and Phase 2.
If you sign up BEFORE OCTOBER 31, 2020 you will be included in the Phase 1 / Phase 2 projects. For Phase 1 implementations, you do NOT PAY any installation costs. Work and funding to achieve our established goals, including free installation, for Phase 2 is presently in progress.
*Modems or special installation costs may apply to certain subscription services.
Boulder Junction’s sign-up campaign is no risk to you. You will not commit to a CenturyLink subscription until you place an order with CenturyLink for fiber to be installed to your home or business and to get High Speed Broadband service (estimated to be in March to July 2021). Signing up now helps CenturyLink understand how successful this project can be for them as well as helping Boulder Junction’s chances of getting another Wisconsin Public Service Grant in 2021 for Phase 2 to get High Speed Broadband to the rest of the Town.
If you DO NOT sign up by the OCTOBER 31st, 2020 deadline you can still get High Speed Internet installed, BUT YOU MAY BE RESPONSIBLE FOR THE INSTALLATION COSTS AND you MAY NOT be included in the Phase 1 / Phase 2 projects. DON'T WAIT!

How to properly fill out the SIGN UP FORM...
Please submit a SEPARATE FORM for each PROPERTY / BUILDING where you want fiber optic cable installed.
For Example - This means, if you own three SEPARATE PROPERTIES or three BUILDINGS on a single property, and want fiber to EACH PROPERTY / BUILDING, you will need to fill out three SEPARATE FORMS
Please keep in mind filling out multiple forms will require a SEPARATE CENTURYLINK SERVICE ACCOUNT and BILL for each property you fill a form out for.
To fill out multiple forms, simply use this form again and fill out a different address.
If you do not currently own the property you are signing up for, you will have to get permission and coordinate installation and sign up with the property owner.
